• If there is any duplication of the same address/ Mobile Number, the form will automatically get rejected.

• We will be distributing only one DVD & prasad box per family.

• Please co-operate and make only one person fill the form on behalf of the entire family. 

• Please mention your phone number and email address correctly.

• Please mention your complete postal address with pin code and provide a landmark for ease.

If the address is beyond the delivery partners reach, it will get cancelled without intimation.
